Registration
All entries MUST be accompanied by a registration form and payment in full. Dance entry prices are listed on the Individual Entry Forms. All sales are final.

Tickets, programs and materials will be available for pick-up at the Fantasy Ball Hospitality Desk outside the Grand Ballroom, beginning 6pm the evening before the event.

From the list below, you can download and print all of the entry forms. The forms are in PDF format and you will need a PDF Reader. There is a link to the free reader below.
If you use these forms, you must send in full payment along with each form.
For credit card payment, a 4% processing fee will be applied; you must complete the Credit Card Authorization form and submit with your registration.
